The urine protein-to-creatinine ratio (UPCR) is a diagnostic test that measures the concentration of protein and creatinine in a single urine sample to estimate the amount of protein excreted in the urine over 24 hours. This ratio provides a reliable and convenient method for assessing kidney function and detecting kidney disease.

Proteinuria, or the presence of excess protein in the urine, is a common indicator of kidney damage. Normally, kidneys filter waste while retaining essential proteins in the bloodstream. However, when the kidneys are damaged or diseased, they may allow proteins to leak into the urine. The UPCR is calculated by dividing the urine protein concentration (mg/dL) by the urine creatinine concentration (mg/dL). Creatinine is a waste product from muscle metabolism, and its relatively constant excretion rate makes it a useful reference for assessing protein levels.

A high UPCR indicates significant proteinuria, suggesting potential kidney damage or conditions such as glomerulonephritis, diabetic nephropathy, or hypertensive nephrosclerosis. Conversely, a low or normal UPCR suggests healthy kidney function with minimal protein leakage.

The UPCR is advantageous because it eliminates the need for a 24-hour urine collection, which can be cumbersome and error-prone. Instead, it uses a random urine sample, providing a quick and reliable assessment.
