The evening cortisol test measures cortisol levels in the blood or saliva during the evening hours. Cortisol is a hormone produced by the adrenal glands that follows a natural daily rhythm, typically reaching its peak in the early morning and gradually declining throughout the day. The evening cortisol test is used to assess this natural decline and diagnose disorders related to abnormal cortisol production.

Elevated evening cortisol levels can indicate conditions like Cushing's syndrome, where cortisol remains high throughout the day due to overproduction. Conversely, low evening cortisol levels may suggest adrenal insufficiency, such as Addison's disease, where cortisol production is insufficient. The test can also help evaluate the function of the hypothalamic-pituitary-adrenal (HPA) axis, which regulates cortisol production. Accurate measurement of evening cortisol is essential for diagnosing and managing disorders of the adrenal glands and ensuring appropriate treatment strategies.
